Registration

Online registration for the 2012 Colloquium is open. Registration includes access to all presentations plus 3 breakfasts, 2 lunches, daily snack breaks, Sunday Reception, and Monday night event. Payment can be made either by check or by credit card. No purchase orders will be accepted this year. For those opting to pay by check, the check should be made out to CISSE, and mailed to:

Registrations will be considered complete once full payment has been received.

Conference Registration dates and prices:

• Up through March 31st, 2012 - Early Bird registration - $475 for full conference
• From April 1st through June 13 - $575 for full conference
• One-day rate - $175
• CAE Recognition Ceremony and Luncheon (June 11th, 12:15-2:00 pm) - $40 (Note: Luncheon is included in all full conference registrations) 

Special Dietary Requirements

On the Colloquium registration page, you have the opportunity of indicating whether or not you have special dietary requirements.  For those who specify vegetarian or low-sodium diets, every effort will be made to accommodate your needs. Contact Dr. Belon for further information.

Conference Refund Policy

The following refund policy applies to all registrations for the annual Colloquium. To cancel your registration and request a refund, please contact the Colloquium Registrar, Dr. Barbara Belon.

• Colloquium registrations cancelled prior to April 15th - 100% refund.
• Colloquium registrations cancelled April 16th through May 15th - 50% refund
• Colloquium registrations cancelled after May 15th - no refunds, except for extreme circumstances such as death in the family, hospitalization, military duty, etc.

At any time, registrations can be transferred to another attendee for no charge. Please contact the Colloquium Registrar to advise of the change.
